File A Complaint – Crime Victim Ombudsman

If you are the victim of a crime and are not satisfied by the services you have received and/or feel that your rights as a crime victim have been violated, you may file a complaint with the Crime Victim Ombudsman (CVO) using the following procedure:

  1. Access the complaint form.
  2. Complete the form online and submit it to CVO electronically.
  3. CVO will email you to verify your submission. Your reply email will give the Ombudsman the legal authority to conduct a formal inquiry on your behalf.
  4. You may also click here to print the blank form and complete it by hand. Please date and sign the form and submit it using one of the following options:
