Crime Victim Forms & Applications

South Carolina became a leader in victim advocacy by establishing the first victim assistance program in the nation. Under the leadership of Attorney General Alan Wilson, the state offers information and support to victims and their families as cases make their way through the criminal justice system. We hope the following information will help you to understand the complex judicial process.

You should have received a form from the law enforcement agency once you make a report of the crime that has affected you. This form is used for notification purposes and it not intended as a tool to create any investigation. A crime must be reported to the local law enforcement agency before completing or sending this form to our office.
